Overview:
Reporting to Lead, ICCP (Integrated Community Care Provider)
- Together with ICCP stakeholders (LB AAC, CCMS, HPC+, and other entities managers) and community partners to support ICCP framework implementation and on-going successful operations of ICCP Model
- Work with Care Organizations Partnerswithin ICCP sub-regions to collaborate and maintain on-going partnership.
- Integrate SOP / Care-Plan workflow with stakeholders and partners, refine and update the workflow as required; document all SOP and workflows based on outcome of ICCP implementation and subsequent operations.
- Support Lead ICCP in Communications and liaisons with internal and external ICCP stakeholders on on-going basis.
- Point-of-contact support for all data and status (eg care-plan) collection platform and tools
