You will provide pharmaceutical care by working collaboratively with a multidisciplinary health care team to promote wellness, maintain current health, and/or intervene in acute or chronic illness.
Responsibilities:
- Recommend and monitor patient drug therapy and provide drug information.
- Identify, resolve and prevent drug-related problems.
- Interact with patients, family, and other health care providers to assure continuity of services and to achieve desired patient outcomes.
- ensure the accurate and timely distribution of pharmaceuticals, and provide education to patients, staff and other health care practitioners.
Job Requirements
- Degree/Master Graduate from a School of Pharmacy recognised by the Singapore Pharmacy Council
- Pre-Reg turning Full registration are welcomed to apply
- Full registration with the Singapore Pharmacy Council.
- Valid membership with local professional body/association is preferred where applicable.
- Experience as a Pharmacist in relevant clinical settings and demonstrated competency will be an advantage.
- Working Hours: Normal Office Hours - 8.30am to 6.00pm, will be required to work half a Saturday from 8.30am to 12.30pm once every 4 weeks
